Test did not ring to dispatch. Talked to PW Supervisor Gerald & they put SCADA online and it did not ring through. Told Gerald to leave the system operational and monitored.

1) No address. Address on the permit is not the address called into Dispatch. PW Supervisor Gerald Arredono needs to work it out with Mansfield and Mareva. Dispatch receives the call as 4535 Sunnyside Rd SE.
2) ROC needed in TCE when address is resolved.

4) FACP sign needed on door
5) Pre-test docs needed in doc box.
